How to Get All Followers in Mad Island
Basic Overview

You will spawn on the gentle shores of the beach as marked by the blue flag. This is a very serene starting region of the game. But the further interior of the island has progressively dire challenges. The deeper you go, the foes become more potent, and they become much more aggressive and harder. Together with the more potent foes come growing bosses that you must slay in order to advance. But of course, in order to build a base successfully and unlock new crafting capabilities, you will be seeking the keys—valuable key items that can only be found by reading the books containing said important information. To unlock the final pieces of the game, the player has to connect the various rare gems and manage to bring down a certain powerful late-game enemy. The fog, which is mysterious in the island, bears many events; while those with green fog can be navigated by either of the two characters, the pink and blue fog can only be travelled by Yona and the boy, respectively.

Reika and Keigo

Activating the event at the beach, surrounded by Rei, Reika, and Takumi, requires bypassing the Old Man blocking the way to the beach, either by going through or defeating him, as well as an exploit in the game known as the flag skip.

Head east from starting beach, continue over the bridge and forward to a tent; defeat the Old Man for the Man Bench Recipe.

Upon defeat the Old Man, walk towards the north of his tent; a scene should be activated with Reika and Keigo. The player can choose to either "Observe" or "Scream" for the scene. This does not alter the quest the player is on, but provides some color to the scene that follows. In either case, Takumi interrupts.

After you end up in the swamp again, stick to the right until you get the next event. If you chose "Observe", it means there'll be a scene and then Keigo breaks up with Reika, but again Reika and Keigo are going to have an arguments in the event immediately afterwards.

Takumi does worse and will run away wwhen he loses the fight. This occurs only if "Observe" was used before the fight. At this point Keigo and Reika will have an argument among themselves and would offer the player to be on 'the side of one particular character or the other'.

Choosing Keigo closes off the romantic option with the Female MC, ending their romance route. If the player chooses to stay with Reika, the two eventually make up and then trigger another event south of the camp. The two players may choose to either let the reunion happen or intervene, going back into the questline of the Male MC.

For the Male MC Romance:

After releasing Reika and Keigo, change to the Male MC and return to that same beach where the Female MC fought Takumi. A blue dot will indicate a new scene with Reika at that location. Because this is a conversation affinity event with Reika, affinity with her should be raised at least to a basic level.

When Reika tells the Male MC how upset she is that he dumped her, the Male MC can choose to pounce on Keigo or to comfort Reika. Pouncing on Keigo will break the chain, and comforting will advance it. Click "Yes" or "No" to back the player out of the text, and "I Love Reika" to bring the player to another confrontation by Keigo.

Picking either "I Love Reika" or "I just wanna f* her" provokes a fight against Keigo. Beat Keigo, and depending on your option after this fight, either to insult or confess to Reika, is the result. **

Picking "I Love Reika" begins the ending observation, where the results show Reika making a permanent breakup with Keigo, realized at the end with the Male MC in a romantic scene. Picking "I just wanna k*ll her" results in a breakup with both the characters; the quest line will finish.

However, after the defeat of Keigo in any route, Reika will ask what will become of him. Regardless of which choice the Male MC Selects, Reika does not believe she is insulted and that it will now be possible to do romance with Reika. In the end, Keigo will give the player his locket, which gives an additional damage.

Takumi

To recruit Takumi: Finish his quest and he is available for a rematch in his private quarters, to the north of where you initially find him. Reiterating, the two options above both lead to the same dialogue skip. Then your final choices are:

You'll require a Steak Set to be able to interact with Shino; create one by combining one Raw Beeff, one Onion, one Potato, and one Berry. Her camp is nestled right into the small alcove at the base of the mound to the northeast, just where the grass begins to turn blue—you can expect your first real encounter with Shino to take place here.

After this conversation, Shino will move to the north mound. You can track her movement on the map. When she's there, come back to her camp during the night to see the next phase of your interaction.

Now, your choices will lead to different outcomes:

Would you like working more on the improvement of conversation, or anything else for this game?

Giantess

The Chaos Wastes are unlocked by three sure gems, dropped by three bosses whom you can know only by killing them. After the Gate is unlocked, you have access to this world where very high-level creatures live. However, most of them you can easily skip, since the last boss really engages your wits. There are three ways to defeat her: You may either befriend her, which forbids romance; enslave her; or kill her. Towards this end, the timing to pursue romance occurs at a certain point during an action in the cutscene after the game's final battle. A word of warning, the Giant enemy that exists within the Chaos Wastes are very dangerous and inflict tremendously massive damage.

Sally (and Takumi?)

To access the Bandage Cave and the final fight, the player must first access the Gate to the Chaos Wastes (albeit making the proper decisions and allowing Takumi to reach said Gate), then a new event will appear on the overworld that will open the cave. It will be centered on a cutscene with male protagonist Yona, Reika, and Keigo if he was allowed to live during his boss battle. Again, an event currently situated in this spot says "Under Development." It is temporary and can be ignored.

Inside the Bandage Cave, head north to the center with the lighter ground. Then turn right, then find the bottom prison door located in the wall. Inside this door is Sally's room. During the fight, you can let Sally go without a fight to be her friend, or punish her. The Punish option gives two more options: kill Sally or confine her in the cave.

Mer-Ooze

You need to craft a boat, fishing rod, and a shell bra and should have at least one worm. She can be found at the most right bottom part of the map to the southeast of the river canal. Upon visiting, dive until you get a rainbow coral and craft a rainbow-shell bra. Upon meeting the mermaid, go to the island beside the mermaid and fish for her lost bra. This will trigger a special scene where you have to play a fishing minigame. Befriending her requires giving her the crafted rainbow seashell bra, or she will simply disappear.
